What To Expect

You can expect to meet with a clinical pharmacist and student pharmacists.They will assist you in managing your health and wellness. You will be provided with education about your health and how to use your medicines. Suggestions will be offered on ways to help keep you feeling your best.

Limited physical assessment (such as blood pressure)may be performed to make sure your medicine is controlling your condition.

Medical history, lifestyle and farm/ranch work, any prescription or over-the-counter medicines or herbal supplements you may be taking may be discussed.

We encourage you to ask us any questions you may have about your medicines and health conditions.

Please keep in mind that this service is in addition to your regular check up with your family physician, not to replace it.

What to bring to your appointment

In order to help us get a good idea of how we can best help you, we need you to have the following on hand for your Pharm to Farm assessment:

  • All medicines (over- the- counter & prescriptions)
  • Herbal Supplements
  • Blood glucose log
  • Blood glucose meter
  • Blood pressure log
  • Weight log
  • Meal and/or exercise diary
  • Any medical records or lab results you would like us to review

By having the above list available, we will be able to get the full picture of your medicine use. This is a customized visit that will be tailored to your specific health care needs.
